A staff member will take some basic demographic info and send you a link to the patient portal. The process is available during business hours. The link is available 24/7. You must be registered first.

FYI:
If you are new to services here, we will do a CCA or comprehensive clinical assessment prior to all services including Counseling, Psychiatric Services, and Psychological Services. If you are actively involved with clinical services elsewhere and are a psychiatry referral, we will need your clinical home to send us a recent CCA. If you are a referral for a psychological assessment, we will need to do a CCA and/or update to gather needed information and releases prior to conducting psychological testing and providing a cost estimate.
